Tulsa Police issued a Silver Alert for a 63-year-old male on Sept. 5.
Police said Terry Chambers was last seen near East 11th and South Lewis around noon, September 5th.
He is 5'6", 165lbs, gray hair, and suffers from Alzheimer's.
Chambers was last seen wearing a gray shirt with blue sleeves and blue plaid pants.
