Police say, during the attempt to flee the scene, Hernandez caused a second collision when he struck a Chevrolet Hatchback head-on.

A woman driving the Hatchback sustained serious injuries and later died from her injuries at a local hospital, police say.

Hernandez was taken to a local hospital for non-life threatening injuries and was transported to the Broken Arrow City Jail and booked on charges of Driving Under the Influence - Alcohol, Manslaughter, and additional traffic related charges.

Police say the release of the woman's name is pending the notification of family members.
