TULSA — UPDATE: Broken Arrow police identified the woman that was killed in a head-on collision early Tuesday morning.

Police say 46-year-old Jennifer Holt of Sand Springs passed away from injuries sustained when Hector Manuel Hernandez struck her Chevrolet Hatchback head-on.

Broken Arrow police arrested a driver after they say he caused two collisions, one resulting in the death of a woman.

In the early morning hours on Nov. 26, Broken Arrow police responded to a "collision with injuries" call.

Police say the investigation revealed that two vehicles had been in a collision near East 81st Street and South Garnett Road.

The driver of the sedan, 35-year-old Hector Manuel Hernandez, fled the scene and continued down Garnett Road, police say.

Police say, during the attempt to flee the scene, Hernandez caused a second collision when he struck a Chevrolet Hatchback head-on.

A woman driving the Hatchback sustained serious injuries and later died from her injuries at a local hospital, police say.

Hernandez was taken to a local hospital for non-life threatening injuries and was transported to the Broken Arrow City Jail and booked on charges of Driving Under the Influence - Alcohol, Manslaughter, and additional traffic related charges.

Police say the release of the woman's name is pending the notification of family members.
